For your convenience, you can now download the PTA Membership and SEM Contribution Form from the MPE Website and fill it out before you arrive at Open House.  The PTA has set up a “Mustang Express” lane at both the New Family and Returning Family tables.

If you would like to use the “Mustang Express” form and “Mustang Express” lane at Open House, please do the following:

Download and fill out form correctly and completely.

Please type or please write all names clearly and in ink.

Calculate your total due accurately.

We will take Checks or Cash Only.  Please staple the check or cash (in an envelope with name on it) to your form.  We cannot accept Credit Cards for the “Mustang Express”.
